Becoming an official PHAP Member
As soon as you have decided on a health profession that is right for you, you will want to join your colleagues in the PHAP. To become an active member in the program whether you are an undergraduate or a postbac (2nd bachelor's), you will need to print and fill out a registration form and pay a one-time non-refundable registration fee amounting to $125.00. The completed form and payment in form of a check (payable to CSUEB) must be mailed or dropped off at the PHAP office. Once you become an active member you will receive a PHAP Student Handbook, you will sign some program forms and your contact will be added onto the PHAP listserv for program updates. The information below will assist those in need of a partial fee waiver.
PHAP Partial Fee Waiver Program
It is the goal of PHAP to assist all students planning to apply to health professional schools with all services we offer. To make this easier, we provide a partial registration fee waiver (for eligible students with demonstrated financially need). All registration fee requests must be received and processed before you can be considered an official member of PHAP.
Depending on the financial need, a student may request a partial waiver by filling out a form (and writing a brief letter of request) in which case a portion of the registration fees is waived. Typically, the maximum amount on the partial waiver is $75 if approved, with the student paying the remaining balance of $50. Processing of the fee waiver takes about a week from the date of receipt of your request. If further information is required, the student will be notified through the e-mail listed on the form.
