How to Use Our Services
To Begin, Register with SDRC at an Intake Appointment
Students requesting accommodations for the first time must complete an intake interview with an SDRC counselor. Call, e-mail or visit SDRC to request an intake appointment.
Prior to the intake appointment you will be asked to provide documentation on your disability which should include information on how it impacts your ability to learn (i.e. functional limitation). Documentation must be from a competent authority. Also, during this intake appointment you can provide additional information about your disability.
Based on this documentation, and their discussion with you, your SDRC counselor will review and identify accommodation possibilities. A "Determination of Accommodations" form will be issued which will state your approved SDRC accommodations.
Only accommodations specified on the Determination of Accommodations form will be rendered. If needs change, make an appointment with an SDRC counselor to review the accommodations. An update to the form will be made if changes are warranted.
For students new to Cal State East Bay, an intake appointment should be scheduled as soon as notification of acceptance to the university has been received. Documentation of each disability should be submitted prior to the intake appointment. To arrange for an intake appointment, please contact SDRC.
Updating Your Accommodations Each Quarter
After your initial intake interview and at the start of each new quarter, you must renew your accommodation request by filling out an Accommodations Request form. Each quarter, a completed Accommodations Request Form (ARF) - Microsoft Word Format ; Accommodations Request Form (ARF) - (PDF Format) must be submitted to SDRC before the accommodations can be provided.
The Determination of Accommodation form lists the accommodations for which a student is eligible based on the documented functional limitations and remains in force from quarter to quarter (unless changes are warranted).
An Accommodations Request Form is an updated statement, by the SDRC-registered student, that they are requesting their accommodations for the next quarter.
Meeting with Instructors
We strongly encourage students to schedule a meeting with their instructors early in the quarter to review accommodation needs as indicated on their Determination of Accommodation form.
Accommodations Appeal Policy
If a student requests accommodations that CSUEB does not currently offer or that the counselor has determined not reasonable or appropriate, the student can appeal the decision through the Accommodations Appeal Process. To request a review, the student will need to complete the Accommodations Appeal Request Form.
